This section provides access to full guidance should you or someone else in your area have an accident or incident

Completed forms should be returned to Safety Services via email or post as quickly as possible. The person's line manager, supervisor or local safety adviser should also be notified.

All accidents, incidents and near misses are investigated by the DSE's investigation team. This allows the identification of any root cause with a view to putting measures in place to prevent an incident becoming an accident, and to prevent further injuries.
