For disability support that is available outside work, you can contact your local Adult Care Teams (Adult Social Care). Adult Care teams can assess you for support that you may need in your home, as well as processing Blue Badge applications. Some authorities can also provide support with benefits and most have dedicated sensory teams to assist adults with hearing and visual loss.
